My principal would like us to send surveys home with out kids asking parents for feedback about our classrooms. I am a first year teacher, but I have had positive parent feedback so this doesn't make me overly nervous. However, I have no idea how to set up a survey like this. Does anyone else have to do one so I could see an example? Or does anyone have any ideas of question/ ways to set it up? I would really appreciate any help. Thanks

I would choose an area that you really want to know about. Last year I wanted parent feedback about communication because it was my first year in third grade. I created my own and sent it out. Truthfully, I was all ready to be slammed and was very pleasantly surprised when I got mostly positive comments. The communication tool that I was worried about was the favoite among my parents.

I attached the survey for you.

i have 2/3 that i send out:
*1 is for the kids at first quarter's end (does your teacher explain things a different way when you don't understand/does she use visual aids...)
*another is a "teacher's report card" at quarter's end for parents (rate my communication w/you; rate my response to your concerns...)
*and the last is an end-of-year survey for parents: (tell me your thoughts about homework/planners/discipline system....)

i'm at home and don't have access to those, and i'm taking a personal day tomorrow--wahoo! so i'll check back on friday at school to attach.

Attached is a copy of our parent survey. I erased the name of our school that was at the top of the first two pages. I also put in the words school or our school wherever it was named. I hope it helps you.
