I'm in the middle school and teach SLD and DCD (Developmental Cognitive Delays).
Our school Psych give a cognitive test - either the WISC, Leiter or Woodcock-Johnson. The Leiter is nonverbal, so sometimes she will give the WISC and then give the Leiter too.
I give the Woodcock-Johnson Tests of Achievement. For DCD kids I give some type of functional assessment, like the Vineland.
We also have to assess transition. We have developed our own Transition surveys that we use.
Speech, OT, PT, etc. have their own set of assessments that they use.
Also, I have the nurse do vision and hearing screenings.
I also do classroom observations, parent interviews, file review, and some informal assessments.
It all depends on the child and what is decided at the Evaluation Determination Meeting.
If there are behavior or emotional concerns, then we will give some assessments in that area.
For SLD, we also have information processing checklists that are completed by classroom teachers.

Depends on the age of the student, cognitive functioning etc. In my district 3 year re-eval and intitial testing is done by our diagnostic teacher. She uses the Woodcock-Johnson for kids over 5 usually. I'm more familiar with testing for EC kids, so I'd use the Mullens or LAP-D.
Speech is done by the speech path, but I've given the Goldman-Fristoe Artic test (D/HH).
