Our district does similar- but the form is only 3 questions, and once sent in, if any of the questions are marked yes- it is red flagged and the principal gets an alert from the system.

TBH- you might to ask that the school stop if the district is doing it - too much PITA and waste of time to do both.

And at this point in time, the form from the district has not been sent out yet. I have a meeting at 735.

Ours is a google form. Easy peesy. We could simply add the district personnel to the list of people who could see the info. Someone didn't think this one through.

Haley- if we weren't allowed in the school, we wouldn't fill it out. But as we are allowed to work from home OR school and the form says that.

So, I just realized upon reading this thread that I've forgotten to do our health screener for at least 2 weeks . I made sure to do it every morning when we were in person, but they wanted us to keep filling it out even though we're remote.

I'm now realizing that I did that for maybe the first week and then forgot all about it. So clearly no one is actually looking at it, because no one has asked me where mine is!

WHAT happens if you don't do the district form? I'd be tempted to ignore their request or just cc the one you do for your P.

With all the technology available today you'd think the district could have worked with your IT department to come up with a way for the information to be shared with those who need it. I'm thinking some kind of google form, where the principals only get info from their staff and district can get all the forms, with staff only needing to fill it out once.

With about 150 schools in the district, the communication is a little behind. It seems a fairly simple matter to me for the district to expect individual schools to report rather than taking this on themselves.

Our p is working on it, but until then... Not that it is too difficult, but it is double the work when no one needs it.

Does the district office know that your building is already doing this? It seems to me that if they know, they should work together and find a way for both your administration and the district office to have it. I'd bring it to the attention of your principal and/or someone from the district office and see if they can work on this. It may mean you have to do both forms for a little bit, but maybe eventually they can make it so both building and district administrators have access to it.

Until then, or if it never happens, I'd probably have fun with the one with the one for my principal, being really sarcastic and smart alecky.

We have been filling out an electronic form every single day since school started this year so our admin knows whether we will be in the building or working from home, plus about 10 questions about our health. No worries.

Now the district is getting in on the action and sent out a note that starting tomorrow we will be filling out a similar thing for the district. Our admin does not have access to that information and is telling us we have to fill them both out now.

What a pain! I certainly feel more protected with my admin (well, the nurse) attending to staff's follow through with the forms and health. Who is going to be monitoring it at the district level? We are a large district with many schoos. No one there will be watching that information.
