We only have at best 15 min. at each center so we keep it short and sweet.

I have them do it on an index card and they don't write the above, they just answer them.

my electronic copy but all i did was just type up a half sheet paper for the kids to use to respond to listening center (would work for library center to).

I had a line on the top of the half sheet for name. "i read/listened to the book _____" "My favorite part of the story was _______ (if you have them reading non-fiction you could change the last sentence to "I learned _____" "I'm still curious about _____" "the most interesting thing to me was ___" depending on what you want to get out of them.

i teach kindergarten so the blank space at the bottom of the page i left for them to draw a picture of their favorite part of the story or to go with what they wrote.

Does anyone have a sheet to help with accountability in the Library Center? I would like something students could fill out in a couple minutes so they know they are being held accountable for actually reading. Thanks!
