I've had classes and kids like this. And an unhelpful principal. It's really tough.
If you can show that his reading is suffering due to his behaviors, wouldn't that be enough to get him help? Even if the guardians don't want him labeled, going through RtI to demonstrate how behavior impacts academics would get him some help in our district. From what you describe, in my school he would qualify for small group reading instruction, and possibly math intervention (we also have some math "business volunteers" who pull kids out for additional help). This might give him the attention he craves/needs and get him out of the classroom at specified times so you can focus on others.
