Found out the reference form someone needs to fill out on me consists of questions such as:

Date of Hire
Would you rehire
Reason for leaving
Questions about my pay
Then a list of personal qualities and whether they are excellent, good, etc, such as:
Attendance
Amount of work completed/Quality of work
Ability to work with co-workers, supervisors
etc, etc, etc

Who can fill out this form since I'm a sub? The principals don't hire/rehire so to speak (even if their word is considered), or know my date of employment, but they may know about my work qualities (only somewhat, teachers would know better about qualities like quantity and quality of the work completed). The superintendent's office would know about hiring and rehiring, my pay, but they won't know my personal qualities. Unless I'm wrong about this?

What would you do with a form like this? I have one main principal where I sub most of the time, and they already wrote me a reference letter, so I need a different person for the form. I don't know the principals at the other schools. There's no clear person I can think of who can fill it completely out. Any suggestions would be appreciated!

I don't think they designed the form with sub teachers in mind. They designed a form for somebody who works at the same place over and over again...

Don't you have some other job besides sub work?

Is there a person or people in your district in charge of subs? In mine there are 2. One to handle day-to-day scheduling issues and questions subs have, and one to handle admin work and such for subs (like licensing renewals, FBI/BCI renewals, etc. Either could fill out the form because both can access personnel files. While they have no first-hand experience of seeing how I work, they base the referral off of whether I'm a sub in good standing or not i.e ever been banned from a school, ever had long term assignments, any positive or negative feedback from principals on file. So you might want to see if your district does that.

And I agree on other jobs. Is it possible to get references from prior employers?

Do you have an administrative assistant in any of the schools who knows you fairly well & calls you a lot?

They seem to know all that stuff!

How about filling in all the general answers (date of hire, etc) yourself, and asking the p you worked for to fill out the rest?

I agree with pp about filling in some of those answers and then asking a principal or a teacher you sub for to fill out the rest. It also probably wouldn't be a big deal if the person just left the first few blank and instead left a comment for how long they've known you.
