What do you use to assess progress in reading or math, and how often do you assess?

What tool do you use to keep track of your progress monitoring?

Reading: We collect data on all students 3 times annually. Between those 3 data collections, we monitor progress on individuals as needed.

Reading data collected in the fall, midyear, and spring includes DIBELS (for students in grades 2-5, it's just the oral reading fluency for DIBELS; K-1 includes other subtests), Scholastic Reading Inventory (SRI-computer-based adaptive assessment that yields a lexile), and the average of all reading comprehension weekly assessment scores (using the weekly assessments published with our reading series). We also track students' mastery of Essential Learner Outcomes (ELO's) as evidenced by PLC created common assessments.
DRA is administered for 1st graders and anyone else needing further evaluation.

We don't do such a good job with monitoring math progress.

All students kindergarten to 4th grade - MAPS - three times per year (Sept., Jan., and May) Students below 24% on MAPS and reading two guided reading levels below their grade level are placed in tier 2 intervention.

All students - Fountas and Pinnell - three times per year (Sept., Jan., and Dec.)

Students attending tier 2 intervention are weekly monitored using STARR.

Students using Leveled Literacy Intervention as their reading intervention materials are tested every sixth story using LLI's reading tests.

I use Curriculum Based Measurements CBMs for reading and math.
These are done weekly or biweekly.
The other types of measurement such as those others listed are used as well.

I'm a title 1 reading teacher and I'm currently using easycbm. They also have a math component. Fluency is every two weeks and comprehension vocabulary is about every 4 weeks.

I use mClass for both reading and math to progress monitor my students. I only work with kindergarten (the school I am at has a Title I aide for grades K-2).
And mClass is really good about keeping track of progress and when the students were assessed.
