Behavior Chart

Clip to ScrapBook #1

I have a student who is very unfocussed. He is quite inattentive, but I want to help him.

I would like to start a daily behavior form for him. I am thinking of a red light, green light type thing. It would be on piece of paper and he and I could color it as the day goes along.

Does anyone know of something like this I could print.

I don't want him to start thinking of himself as "red" but I want to report to his parents and most importantly, help keep him focussed.

Please share. The simpler the better as I already have two others on behavior charts that take up a lot of time/energy.

I used die cut paper shapes and awarded hourly or once a day. This worked for many years and many grade levels. It was a very efficient method to inform parents of how the day went.
