I made a sub report template that I have TRIED to attach. It doesn't look like anything is attached, so I'm just checking to see if it did, indeed, attach.

p.s. It is nothing fancy, but it worked yesterday when I used it for the first time.

Well, it looks like it did attached, but since I'm completely computer illiterate, I can't open it, I think (not sure b/c there are links on the bottom of this screen?). Anyway, if you can open it, it works, though nothing fancy, I assure you.

Similar to the one I use. I keep it simple and short. As a retired teacher, I hated coming back to reams and reams of reports. Made me wonder when the sub actually took time to teach.

I usually write on the lesson plans unless there is a compelling reason to write a longer note.

Short and sweet, just as it should be. I only sub in elementary, so I normally write on the lesson plans, but Id use your form if I subbed in the upper grades.

I would love to have something a little bit fancier than what I currently use----first-grade handwriting lined paper! I have a bulk box of it my son never used, so what not? Hahahaa!
