I'd also recommend Debbie Miller's book, Reading With Meaning, for grades K-2 and Strategies That Work by Steph Harvey and Ann Goudvis for grades 3-5.

Strategies That Work by Stepanie Harvey
The Struggling Reader by J. David Cooper, David J. Cahrd, and Nancy D. Kiger. I also liked Mosaic of Thought.
