A few years ago, my teammate showed a 6th grade boy her service log and told him she had to "report to the government" when he refused to work with her. Unbelievably, it worked and that scared him into coming. Worth a shot!
Since you've already tried rewards, I'd try consequences at this point. My go-to would be, "It's not a choice. You can come now or at recess." Presumably you've talked to him and tried to work out any actual issues there may be. Last year I had a kid who would throw a fit every time I came to the door, but was 100% fine as soon as we stepped into the hallway. He didn't really mind coming, but was embarrassed and wanted to "save face" with his friends. The class he was in was a Lord of the Flies nightmare, so it just wasn't a battle I fought. The classroom teacher had far more pressing issues.
I'm also wondering if you've contacted parents? Either they can help you get the kid on board, or if they can deny services if they really don't care that he doesn't go.
