What information about assessments do you send/receive about your students at promotion time?

I can think of things like reading levels (GR, DR, AR, etc), or maybe even the forms from these assessments, but what is really helpful for/to other teachers?

I've received a mish mash of information before, including work samples (which I don't find helpful). I want to know what a kid has been introduced to (because we all know we don't get to everything with everyone) and what they have mastered (just because we teach it doesn't mean they learn it). Is there an easy way to get this information(because it isn't in letter grades)?

What does your school do?

At my school, we give the next year teacher their reading level, if they have been CSAPed, ESOl, and math level. That is al; we give them.

We have Principal Cards that go with the students year to year. On it we list their levels-below grade level, at, and above level for the different subjects. Then the state test scores if they are above 3rd grade. K-2 has DRA scores, 2nd and up list their Lexile scores. I know when I did third grade the 2nd grade test scores were inaccurate-a kid would be proficient but could not add two number or spell. Either that or they lost all ability over the summer!
