Help with a student and fractions

I have a 5th grade student who is struggling with fractions - adding and subtracting mixed numbers and fractions. She draws a blank when trying to find a common denominator, or subtract from a whole number and having to borrow. I have worked bit by bit with her, using a whiteboard, using diagrams and pictures, using manipulatives, but she still doesn't get it. I don't know what else to do to help her. I'm afraid that she has just shut down because it's too hard. We had a test last week, and she didn't even bother working out the problems...just wrote down random answers or picked random multiple choice answers.
Any suggestions, ideas, etc. would be very welcomed. I have never experienced this level of non-understanding before.
