How many of you require your students to complete reading logs each week? If you do, what all is included in your log?

My students are required to read 30 mins per night. To show that they read, the students are to complete one response on a 5 part response log. On Mondays they get the log, which has 5 spaces for different responses (ie: summary, flow map, connection, visualization, etc...) They choose one per night to complete. For the response, they need to write the title, author, pages read, and the date. Then the response required underneath. Each day it comes back to be checked off, then returned so that the next day the student may do another response based on 30 more mins of reading. By Friday, 4 of the 5 choices are filled in. It is the turned in for a completion grade as part of their homework.

ETA: I am attaching the reading log I created (since I was asked for it in a PM, and can't attach things in there!) When I copy the logs, I usually mix and match the 3 section ones with the 2 section ones, so that the kids get a variety of required responses.

I assign 30 minutes of independent reading on weeknights and 60 minutes of independent reading on the weekend. Each night, students are responsible for filling out a reading log. On the reading log, they record the title of the book, the author of the book, the genre of the book, their start page, their end page, and the number of minutes they read (many of my students read for longer than 30 minutes). I also require students to get a signature from a parent or guardian each night.

school has to do a nightly reading log. It is printed for the entire year. They have to write the Title, number of pages read, and get a parent signature. That's it. You never truly know if they are reading the books until they take their AR tests on them. Even then you don't know if they really are reading the books.
