Title 1 record keeping

I am a Title 1 Reading/Math teacher in grades 1-5. I do primarily a pull out. Does anyone have good easy record keeping ideas to show progress or lack of besides district assessments. We are in early stages of RTI and don't have anything "official". I was thinking of a notebook for each child, but don't know what to put in it. I only have 30 minutes with each group and find it hard to assess everyone. I would like something that is not specific to their basal series. It could be weekly or monthy. Something more than 2 times a year district stuff that I have to wait until scores come back weeks later. I just feel like I need more documentation, but don't know where to start. I work with the basal, but also do high frequency words, most often mispelled words etc. In math I need something for math facts and skills like time, money etc. TIA.

I'm a Title I reading specialist, and we use PALs quick checks for grades K-3:

I have also developed/borrowed/stolen a Dolch word list and flash card set for my records. I've attached it my records sheet, and here's the link for the flash cards and sentences for each word:

For grades 4, 5 we conduct QRIs in the fall and 3rd quarter to record progress:

I also do running records at least every couple weeks because they're easy and quick.
