Math Extended Response

Basically any multi-step problem will do. I just use the Mailbox 5th grade problem solving book for examples of problems. Make sure you also have them practice setting up graphs and writing about how they set them up. That has come up several times. Also, ISAT loves mean, mode, median, and range so have the kids do a few problems practicing those skills in extended response.
I wouldn't spend too much time on the actual writing out of the extended response- it just doesn't count for that much of their math test score. Instead I spend time on problem solving and showing their work as they problem solve. This will help them throughout the test. I have a graphic organizer I use to teach the explanation part of the exended response that works really well with my kids that I am attaching. Almost all of my kids (I do however teach the highest math group in our small school) receive 4's for explanation.
By the way, make sure you hit geometry hard. We have found that there is always a ton of geometry questions on the ISAT test.
