The instructions throughout the C4 mention only four types of sources for KOS:

you gathered information from at least two of the following sources: families, colleagues, professionals in the district or in the field, and/or other community members.
I already described three sources: Ellevation (website for data for ELLs), former teachers, and families (parent survey).
However, I feel like I could include student surveys, my own observational notes, and my own pre-tests to add to the description of students. Would these be considered good sources for KOS? Or should I use them for the assessment/SN/PLN sections?

You've got a pretty good set of sources with what you've described, and they cover the ones suggested by the NB, but it's ok to add others. Surveys, pretests, notes are always helpful, regardless of whether one is doing the NB process or not. You might also consider looking at school records for testing data, attendance etc.

The KOS must weave its way through all the other sections, so whatever data you collect, you can use it as one of the criteria for the instructional decisions and choices you make.
