Iím looking to revamp my parent questionnaire to get to know their child from the parentís point of view.
If you give such a thing out, what are some questions you ask?

I used to do this. Here are some of my questions in random order as I think of them:

Who does your child live with?
Tell me something wonderful about your child.
What is your parenting style?
Generally, where does your child go after school/who picks them up?
Does your child have any health issues or allergies I should know about?
Have there been any recent changes or big events in your family lately that might affect your child?

Some of this is rather personal so be sure to emphasize this is a confidential, voluntary questionnaire. That said, Iíve never had a parent not fill it out. The parenting style question results in some very interesting comments.

Here are a few of ours:

Who does your child live with? Circle all that apply.
How does your family approach homework?
What motivates your child?
List 4 words to describe your child.
Tell me anything else you'd like me to know!

I like the examples above, and would also include:

What kind of environment is your child is used to / most comfortable in?
........very quiet----light background noise----busy and loud

How would you classify your child as a reader?
........reluctant----typical----enthusiastic

Would you consider your child an introvert or an extrovert?

What has/have previously been your child's favorite school subject(s)?

What is your child most looking forward to this school year?

I used to do this earlier in my career, but in the past few years I've gone to a different format. At the top of a paper I put: "In a million words or less, tell me what you would like me to know about your child. If you need more room, please use the back of this sheet or staple another sheet to this one."

It's amazing the kind of information you get from this. It can be very informative!

I used to have this in my computer files, but I just retired and purged all my school stuff!
