This year I'd like to do a monthly reading log. I want more than just a calendar where they initial that they read for a certain amount of time. I think there should be some more accountability such as listing the main idea for the story and some details? How much is too much? What would the format look like? As you can figure out, I've never done a monthly reading log, but I think it is important to start one! Any help will be appreciated!
There is a group here for it. It has convinced me that a book log does not encourage at home reading. The author does a book challenge instead. If you teach primary, Christina Bainbridge has a blog that has a challenge she developed. I've been doing all kinds of reading logs for over 20 years, and never had the success I was looking for. So, this year, it's a book challenge for me (and it's voluntary!) I just need to sell it to my team and P.
Here's another idea, a reading log in booklet form that suits those reading chapter books ( print as many pages as you need). It is designed for the child to read six nights per week. Each night there is a brief comment box to complete and when the entire book is finished the child completes one of the 12 response pages about the finished book.

Wow! Thank you all so much for the wonderful ideas. Aussie and 1956 - I love the templates! I also appreciate the Book Whisperer suggestion. I have wanted to read that book forever and maybe now the time has come!
Thank you for taking time to share. My favorite is the one called reading log 3. I like having a area for "response" instead of "summary." I also like that there are examples for ways to respond.
