I'm looking for or wanting to create an exit slip template for my students for the end of each week to see where they they believe they are in their learning. Something along the lines of name 3 new things you learned this week, what is something that you now know very well, what are some things you want to know more...

Does anyone have something like this or use something like this? Any input would be great!

Here's what I've used before. I change it week after week based on issues but come up, but you get the idea.
