Hello...
I'm "stuck"! I'm trying to create an IEP data collection sheet, I can't figure out how I want it done. If you have a copy in Word or Excel, please attach. I need some examples to get my brain flowing. I know... I want a place to mark the date, the objective, a place to code the objective etc...

As a new member you are not able to attach files. If you want to give ILuvSpecEd your email address, she can contact you to get the files, but other than that you are not able to do so.

I use excel document for each type of goal. (reading, writing, behavior, math)

I make a "worksheet" (those tabs at the bottom) for each student on my caseload that has a goal in that area.

I print out a copy to keep track of the data throughout the quarter. I put them in page protectors in a binder and whenever that area is assessed I pull out the data sheet and record the information. (I developed this when teaching 4 inclusion classes.)

Once I have enough data towards the end of the quarter, I plug the information into the computer and average each goal and objective. There's your IEP progress info!

You can also just click and drag to make charts for each goal to show progress =)

I made this last year in the hopes that I'd actually use it, but never did. So, I dont know the effectiveness of it. But feel free to try it, use it, or modify it.

Thanks for sharing your data sheets. I have found that you need more than one kind of data tracking sheet depending on your students' goals. Very Helpful!
