Our school requires us to read one on one with each student each week. I did very well with this last year, but I am looking for a better form to use to document how they are reading. Last year I keep an individual student form which included:

Date/Story Read/Level/Skills Assessed/Comments
Please let me know what you do?

what she used with her K class years ago. I will take a pix at school today to post later. It has been wonderful. Laminate a file folder (I didn't even have time to do that but it can help the folder last longer) Layer large blank index cards on each side of the open folder. You are creating a type of 'flip chart' where you can see your class at-a-glance and not wade through a comp book or 3ring to find their info. Date and note what you did and their next goals-create your own shorthand or symbols. It's awesome. My next step is goal cards to take to their tables since they need to remember 'what' they are doing now (duh)

The one I use for my readers comes from the Making Meaning program and focuses less on reading strategies and more on comprehension/vocab./individual goals. It's at school so I can be more specific after I look at it tomorrow.
