For the 3rd question of the Contextual Information Form, what more do I need to put regarding state mandates? Am I to include the proficiency rate of each grade level? Or am I overthinking this?

What I have so far:

"I teach in a urban school district in the DMV metro area. 92% of students are Black or African American. 8% of students are Hispanic, 2% of students identify as White/Pacific Islander/Other. 95% of students are from low-income households. 76% of students received free or reduced priced meals. 15% of student population are English Language Learners and 14% are students with disabilities. State mandates includes yearly and quarterly assessments in English, Mathematics, and Science. Access to technology is limited and not frequent."

No identifying info....you should not tell assessors where you are. I think this is the DC area? Other than that, you're good.
