I have one that I use. I will try to remember to post a copy of it on here for you. Forgive me if I forget tomorrow, I started graduate school tonight, and the second class is tomorrow.
I tried attaching my activity as a word document but it says the file is invalid! Not sure if you're still looking for something but on the first day I had students fill out a poem with things such as favorite subject, favorite color, favorite food etc. then tomorrow I'm going to have them decorate it and I took their pictures individually and I'm going to tape those on to the bottem of the poem and display them around the classroom. I try to make up a "Silent Walk Around" inventory that actually serves many purposes. I use a 25-box grid and create my own questions or statements such as "I have a pet" "I am an only child" "I traveled on an airplane this summer" until I fill in all boxes. The students are told to look closely at each statement and sign their name on only 1. Then, they walk around the room SILENTLY looking for signatures on each box. The goal is to have every child sign your sheet.
The class is excited to get the signatures, but it also helps you see who can be silent and who has difficulty. When you call Time Up, they sit down and each child can either share one of their own or someone else's answer. This is a great "get acquainted" activity that I do every year.
You may want to write a list of questions for homework that are more in depth for interest inventory. There are many available online.
whtny -- your file may have been done in a version of word the PT software doesn't recognize. I use Vista Word 07...and I always have to change the file to save it as an earlier version (Windows 97-03) You can do that in your document by clicking "Save as..." It should be an option.
