Does anyone have a weekly Homework Template?

Does anyone have a weekly homework template they could share. This year, I would like to do weekly homework.
I need some ideas on how to set this up. I would love to see templates that have worked for you.

Here's one that I use. I think I found it here but I changed things to fit what I do. I type their spelling words in the bottom and then I use this sheet as a cover sheet. I add things that I want them to work on for the week and staple it all together in a packet that I give them on Monday and due back on Friday. I love the weekly packets and so do my parents. It gives them flexibility, yet gives them the opportunity to help their kid and reinforce what we are doing in class. Hope this helps.

This is the homework sheet that I use in my classroom. Each student is given one at the beginning of the week with assignments already included in the template. Knowing what you are going to cover week-to-week makes it easy to pull your homework. I give the assignments out daily because I find that most students will do ALL of them in one day and have nothing else to do for the week.

My template is pretty simple as I typically only assign math and reading each night. It also has room for quick parent-teacher notes, which makes it easier for me.

I use a homework calendar that I found on hellomrsjones.com. You can change it to fit your needs.
