Look up developmental milestones for 3 year old and pick and choose which ones you want to use. Use a variety from all areas and grade on a simple system of emerging, proficient, etc.. Don't forget to use spell check.

I made up my own template based around the domains of development. I made categories of cognitive, social, emotional, and physical, then commented on children's strengths and areas to work on under each. Now that I think about it, there was probably a category for language development, too...or maybe that was under cognitive. It was awhile ago and I can't find the form, sorry.

Anyway, I just wrote it all as a narrative. It kind of took awhile, but IMO it was worth it to give families detailed information.
