Book Report forms anyone

I am looking for a book report form to give to a high level student...anyone have a good one to use for a home report?

that I bought on amazon.com called Independent Reading Management Kit/Grade 1. They come in different grade levels.

It has several book reports that are packets that address not only comprehension items but phonics and grammer things to look for in the book. This book is awesome and my students love doing these reports. Gifted kids really love them. My low students love them too.

Try this site. Click on the Logs. When I found this (and I don't remember how or who I got it from) I got so excited! It is exactly what I have been 'crudely' making-I am really lousy at making snazzy looking papers. What makes this even better is it is no work for me!

I have some good generic forms for different genres of books. They are pretty basic and you could adapt them for whatever level. If you would like to see them pm me your email address and I'll send them to you.
