Anyone know of a good template to use for a vocabulary test for 5th grade?

Here's a sample of what I use. It's just a simple matching and then I make them use some higher level thinking to apply the words (also, it gets the test to an easy 10 points for me to grade! ).

They do it every Friday (or every other depending on how long we spend on each story in the basil).

thanks, I have 14 words to give them plus a crossword puzzle to fill out so not really sure how I would grade that but thanks.

I would make it so I gave them the 14 matching plus pick 6 words to use in the sentence to make it an even 20 (easy grading). I don't know about the crossword puzzle. Why do you give it? As practice? For assessment?

I give the crossword just for fun but I don't have to count that in the test. I will give the 14 matching and pick 6 random words for sentences...good idea. Thanks again!

I really like Scholastic Vocab Quiz Maker website that you posted. The definitions pretty much matched mine from the text.
