Daily 5 lesson format

Hi everyone! I am having difficulties creating my plans for Daily 5. I have always created my own template for my plans in a word document but I seem to keep needing to add another column, row etc. I am a visual learner...is there anyone who does Daily 5 and has a copy of thier lesson plans so that I could see how to create one. I am in my third week of school and it is driving me crazy not having a good plan format. Thank you!

I use www.lessonplanedu.com for lesson planning. This is what my D5 block looks like (blank, no actual lessons)

Daily 5
Read to self
Work on Writing
Word Work
Read with Someone
Read to Class

Lesson 1
Lesson 2
Lesson 3

Round 1
Round 2
Round 3


And then I list next to each what I am doing for lessons & who I meet with during rounds.

I've seen several examples on both here and on other websites. I just use the 'table' section in Word to make what I need. I found that for me, it works best to have one 'general' plan book showing all subjects including Daily 5 mini lessons. I have another book for the small groups and conferences. This has room for the detail I need.
