Would anyone be willing to share the form they use for parent/teacher conference (or what you put on the form if you don't have a copy.)? Also, do you send home a form before the conference? What do you put on there? TIA!

I make a copy for the parent after they attend the conference and sign it. I send it home in our regular communication folders. They do not get a copy before hand. I am afraid they will not come to talk to me if I send it home first.

I thought I had a copy of the file saved on my portable hardrive. I can post a copyof what I use tomorrow if you would like as I have it saved on my computer at school.

My form is on a different computer too. Here is what I have on mine.

Reading:
Fluency: Their initial score and the target score.
AR: Points earned and their percentage correct.

Math:
CBM: Their score and target score. (We use Aimsweb.)

Work and Study Habits:
It is a checklist that I check off.

We also go over their grade card. I usually show an example of each of the fluency and math tests we give so the parents have an idea of the skills. Parent's are also given their student's reading record from AR.

This is the form I use when having conferences. This is the one that I use when I am going to hand write all of the information. I will also post the one that I use that I am going to type up the information. Same layout just with text boxes.
