Seeking Title I Math Progress report

After having written over 50 Narrative type Tittle I math progress reports last year I am looking for a checklist that will make my life easier and inform parents about what is truly important. Does anyone have a good progress report of this type to share? I would love to see some samples of what other people are doing.

This is an awesome idea. I would throw one together if this wasn't going to be my very first cycle of report cards as a Title 1 math teacher myself (I am not sure what kinds of check boxes there will need to be yet). Hope someone has a resource!

I reported quarterly using the rubric of the math program (which is currently Everyday Math). I would use the A-attained, P-Practicing scores and I being an additional program only focused on Part A (focus skills).

I would have three columns as follows:
In Class Unit Content Assisted by Title 1
Progress
Comments on Progress
Write the numbers 1 through 20.
Yes, sometimes reversals on 3’s and 5’s. Used stories to implant a correct image in her mind. (3 has the bump, bump like B. 5 likes to walk with his belly button out and his hat pointing forward.)Make tally marks for numbers.
YesCount forward by 1’s.
Only starting at 1. If asked to count on from a larger number Student has difficulty.Name numbers that come after a given number.
YesName numbers that come before a given number.
We are still practicing this.Understand game of “Penny Plate” (sums of 10)
Still practicingUse a tally chart to answer questions

Calculate and compare the value of penny and nickel coin combinations
Hard time transitioning from counting by 5’s to 1’s. Using a number grid to place the coins will help Student see the counting pattern change. (i.e. 3 nickels and 3 pennies. Place the first nickel on 5, second on 10, third on 15, first penny on 16, second on 17 and last penny on 18 to show the total amount)Show and read time to the hour.
Yes, some reminders to use the short hand for the hour.
