Reading Comprehension and Reading journals

Hi,
I am trying to move my students beyond their regular phonics reading and practice. I have started with leveled readers and want to incorporate reading journals to check for comprehension. Does anyone have some suggestions for the journals? I teach a combined classroom K-3 so I am juggling various levels of reading comprehension.

Thank you,
Wanda

I ask many different things:
What they liked best
Problem/Solution
Summary (upper readers)
Message of story
Realistic fiction, fantasy, or nonfiction how do you know?
Tell about main character
Write a question before reading and after reading answer

My students have a composition notebook in which they write a response to the guided reading level book that was read in small group. I write a question or prompt that relates to the text and the focus comprehension strategy(problem/solutions; sequence of events;compare/contrast;understanding characters). For non-fiction texts I ask the writer to tell what he/she learned about the topic and also the authorís main purpose.
