I am working on entry 3 for MC Gen. Can the instructional materials be actual pieces of work the students did? Instead of including a blank worksheet, I would like to include one filled out by the student.

Yes, I thought it was better to show student completed work. But I think you can show blank sheets.

Can instructional materials include a photo of a chart? You can not see the chart in the video (a nursery rhyme big book). During the video we added post it notes as we discussed the text. Can a I take a picture of this chart and post-its and submit as an instructional resource??

include a picture of the chart. Read your entry notes on size it can be. Also you can add a caption below to let accessors know what it is a picture of and reference in your writing.

Yes, you can use photos of charts or posters. I included some photos for Entry 1 (one was of a plot poster I made, and the other was a photo of kids writing on their graphic organizer during a field trip -- they wrote a personal narrative about a field trip we went on to an art museum.. and I made them carry a clipboard and graphic organizer, and write the sequence of events from the day).

However, I did score the lowest on Entry 1 (I still don't know why?!?! but that God I passed -- whew!), BUT, I don't think my score was low because of my photos. I think using photos makes the entry more interesting and unique!

GOOD LUCK!!!

Your evidence will be stronger if you create the instructional materials yourself, rather than using a worksheet produced by a company. Think about what your goals for learning are and how you can improve just the fill in the blank type of worksheet.
