Do any of you have a student reflection template? Or know of a website with any? I have been searching everywhere and cannot seem to find a good one.

On the "Discipline Without Stress" Yahoo group, in the files, there is a STAR reflection sheet. You have to be a member to open it, but here is the content

Dear Parent:
STOP, THINK, AND ACT RESPONSIBLY referral forms are attached. The purpose of this type of form is to help students understand their self-chosen behavior level and to become more socially responsible. The FIRST referral is handled between the teacher and the student.

If the SECOND referral is necessary, copies are sent home.

Please note that your child has chosen to act on a level that is not acceptable in my classroom. Please have your child explain this level to you.

If a THIRD referral is necessary, copies are sent home, and future socially irresponsible behavior will be referred to the school office.

Please sign this note and have your child return it to me. Thank you.

Have you seen any reflection templates for Reading and Writing?

Ah, you were asking a different question I see. You might want to look for reading responses rather than reflections. If you google that you might find something. For that you might think of looking for Math Problems of the day or Math journaling. I don't have much experience in the latter, and no particular form for the former. I did just purchase a book called Reading Response forms by Eleanor Summers from Classroom Complete Press and How to Report on Books from Evan Moor.
